A survey template to assess attendee’s previous knowledge on gender affirmative practice before participation in the presentation. Gives attendees the ability to provide viable feedback and presenters to evaluate the impact of the presentation. 

In collaboration with MAX Ottawa, the Centretown Community Health Center is offering operational resources to help with the rapid increase of the provision of gender affirming care in the community. Due to the nature of some resources, they are restricted to healthcare providers, requiring registration to access. Once granted access, you will be able to engage with a range of templates and intake stamps to assist with various patient encounters.
