A survey template for learners to complete 12 months after participation in the program to gain information regarding their experiences providing gender-affirmative care within the community. A final survey to assess areas for improvement, key skills and takeaways, comfortability levels and post engagement experiences. 

In collaboration with MAX Ottawa, the Centretown Community Health Center is offering operational resources to help with the rapid increase of the provision of gender affirming care in the community. Due to the nature of some resources, they are restricted to healthcare providers, requiring registration to access. Once granted access, you will be able to engage with a range of templates and intake stamps to assist with various patient encounters.
