A survey template to evaluate increased comfortability and knowledge in the provision of gender affirmative care after participation in the placement program. Gives attendees the capacity to provide viable feedback, as well as demonstrates the impact of involvement and areas of improvement to further increase knowledge acquisition. 

In collaboration with MAX Ottawa, the Centretown Community Health Center is offering operational resources to help with the rapid increase of the provision of gender affirming care in the community. Due to the nature of some resources, they are restricted to healthcare providers, requiring registration to access.
