A survey template for learners to complete before starting placement to gain information regarding time involvement, ability to access gender affirming health services, comfortability and likeliness of provision of gender affirmative care after completion. 

In collaboration with MAX Ottawa, the Centretown Community Health Center is offering operational resources to help with the rapid increase of the provision of gender affirming care in the community. Due to the nature of some resources, they are restricted to healthcare providers, requiring registration to access. Once granted access, you will be able to engage with a range of templates and intake stamps to assist with various patient encounters.
